### Possible causes

* Knowledge sources were temporarily unreachable during refresh cycle
* Specific external knowledge sources were offline or actively syncing

### Possible solutions

* Wait for background synchronization processes to conclude before retrying
* Note that some knowledge data could temporarily reflect an older state
